About The Position

This position serves as a member of the Choices Practice Improvement Team and part of the site-based leadership/management teams to provide site-based training, coaching and support. The position is charged with ensuring fidelity to high fidelity wraparound and care coordination practice models, for Family Preservation families. The incumbent will also participate in site-based and organization- quality assurance/improvement initiatives. The incumbent also supports staff in ensuring best practice within the context of contractual requirement and policies/procedures. This incumbent will also act as a mentor in providing reflective practices to staff through on-going skill development activities. In addition, the incumbent provides on-going support to staff regarding quality review and development of improvement planning.

Requirements

  • Minimum of Bachelor’s degree in Social Work or related human service field is required in most contracts.
  • 2 years of experience in a care coordination role or supervisory role
  • Experience in providing training and/or consultation within the human services
  • Strong knowledge of and genuine respect for youth and adults with mental/behavioral health issues and a firm commitment to empowering families.
  • Strong communication and writing skills.
  • Highly organized, detail
  • Must possess a valid driver’s license in state of residence and auto
  • Demonstrated ability to Work effectively with internal and external individuals, including other professionals in the community.

Nice To Haves

  • Experience in Child Welfare, Juvenile Probation, Wraparound Facilitation and/or other related services preferred.

Responsibilities

  • Provides training to all Family Preservation staff providing high fidelity Wraparound and Care Coordination practice
  • Functions as a reflective practitioner in modeling system of care values and principles of high- fidelity wraparound and care coordination best practice.
  • Collaborates with staff to provide process consultation and assistance with application of practice model to all families working in Wraparound under Family Preservation.
  • Provides direct training and support for staff within their first six months and beyond for individualized learning needs.
  • Provides ongoing coaching/mentoring to initiate and sustain practice.
  • Serves as a consultant for staff to promote practice skills and outcomes related to four key phases: engagement and team preparation, initial plan development, implementation and
  • Understands the needs of adult learners and can be flexible in their approach to engaging and training staff.
  • Provides quality assurance/improvement through on-going practice and processes using key measures and assessment tools.
  • Plans and executes quarterly learning opportunities for staff based on data and information from the quality improvement/assurance process.
  • Participates in Program Quality Improvement (PQI) team planning
  • Assists in training and development of staff through needs assessments in collaboration with
  • Collaborates with corporate and site-based leadership/management to support staff development and facilitate working agreements around training needs.
  • Collaborates with site-based leadership to strengthen Choices’ community presence and understanding of system of care values.
  • May carry a partial caseload in an effort to meet the needs of Choices and to allow for modeling opportunities as needed.
  • Develops and implements training on clinical procedures within the Wraparound process.
  • Reviews and gives feedback on all coaching documentation completed by Supervisors.
  • Willingly completes other duties as assigned to advance the mission of Choices.
