About The Position

We are seeking an experienced and driven Nurse Practitioner (NP) to serve as our Practice Clinical Lead in an on-site leadership role based in Orangeburg, NY. This position combines clinical care, team leadership, operational oversight, and program development. You will play a key role in managing the NP team, ensuring compliance, and driving performance across all clinical programs.

Requirements

Active Nurse Practitioner (NP) license
Willingness to obtain and maintain multi-state licensure (TX, FL, PA required)
Proven leadership or management experience
Strong communication, documentation, and clinical decision-making skills
Ability to work on-site in Orangeburg, NY

Nice To Haves

Experience in CCM, RPM, telehealth, or value-based care preferred

Responsibilities

Provide oversight across CCM, RPM, BHI, and telehealth programs
Supervise and mentor Nurse Practitioners and clinical staff
Ensure compliance with CMS guidelines, HIPAA regulations, and internal standards
Train and onboard new Nurse Practitioners
Provide ongoing coaching and performance management
Foster a high-performance, accountable clinical team
Conduct audits of documentation and clinical workflows
Ensure timely and accurate charting (within 48 hours)
Maintain audit readiness and regulatory compliance
Develop and refine clinical templates and workflows
Collaborate with operations and field leadership
Support rollout of new programs and technologies
Communicate care gaps and findings to partner facilities
Work with interdisciplinary teams (LPNs, care coordinators, facility leadership)
Oversee NP-to-facility assignments and team structure
Conduct approximately 50 virtual patient visits per week
Provide care across CCM, RPM, and BHI programs
Develop individualized care plans
