About The Position

The Practice Administrator provides strategic and operational leadership for CareConnectMD’ s Post-Acute Medical Group across multiple markets. This role is responsible for driving clinical and financial performance, optimizing practice operations, and aligning physician incentives with organizational goals in a value-based care environment. This role is instrumental in defining the mission, goals, budgets, and performance standards for clinic operations and ensuring alignment across physicians, staff, and leadership. This role partners closely with physician leadership to optimize provider performance, implement effective incentive models, and drive outcomes in a value-based care environment. The ideal candidate brings deep experience in medical group operations, strong financial and analytical acumen, and the ability to lead teams to deliver high-quality patient care and exceptional service.

Requirements

  • Bachelor’s degree in Healthcare Administration, Business Administration, or related field (or equivalent experience).
  • 8–10+ years of progressive healthcare operations experience, including medical group or clinic management.
  • Demonstrated experience managing multi-region or multi-site healthcare operations with P&L accountability.
  • Strong knowledge of revenue cycle management, healthcare KPIs, and practice operations.
  • Experience working with physicians and driving provider performance.

  • Experience in value-based care models (ACO, Medicare Advantage).

Responsibilities

  • Provide oversight of medical group operations across multiple regions, ensuring consistency, scalability, and operational excellence.
  • Establish and implement organizational goals, policies, procedures, and operating systems.
  • Standardize workflows, staffing models, and best practices across markets.
  • Lead teams to deliver a high level of patient and customer service across all care settings.
  • Utilize knowledge of organizational systems, policies, and procedures to drive operational effectiveness.
  • Exercise independent judgment, initiative, and problem-solving in a fast-paced, evolving environment.
  • Maintain accountability for operational performance across regions, including staffing models, provider workflows, and care delivery execution.
  • Oversee day-to-day operational performance and ensure effective coordination across interdepartmental functions.
  • Ensure provider coverage standards are met (visits/day/week, admission timeliness and weekend/after-hours coverage).
  • Monitor and manage rounding schedules, documentation timeliness and workflow compliance.
  • Identify and resolve operational gaps impacting care delivery.
  • Establish communication systems with facility staff, triage and providers to improve efficiency and quality.
  • Support execution of initiatives tied to: MSSP, ACO REACH, LEAD.
  • Monitor effectiveness of operational processes and implement continuous improvement initiatives.
  • Ensure appropriate delegation of responsibilities and development of leadership capabilities within the team.
  • Own P&L performance across the medical group.
  • Develop and manage budgets, forecasts, and expense controls.
  • Review monthly financials and implement strategies to improve revenue and manage costs.
  • Monitor key operational performance indicators, including: Provider productivity and schedule utilization, Revenue cycle performance (collections, denial rates, days in A/R), Adjusted collection rates (target ≥95%), Cost management and contribution margin.
  • Track and improve key VBC metrics: Readmissions (RTA), ED utilization, Length of stay (supportive, not primary driver).
  • Ensure alignment of expenditures with budget.
  • Partner closely with Revenue Cycle Management (RCM) to optimize billing, collections, and denial management.
  • Monitor collections performance and provide regular reporting to leadership.
  • Ensure accurate charge capture, coding alignment, and compliance with payer requirements.
  • Maintain strong understanding of Medicare, Medicaid, and managed care guidelines.
  • Manage SNFist physicians and advanced practice providers (NPs/PAs) schedules and staffing.
  • Track and enforce: Productivity (visits per day, admissions), Quality metrics (RTA, care transitions, documentation quality), Compliance with client requirements.
  • Provide real-time coaching on: Facility engagement and communication, Family interaction and care planning.
  • Partner with clinical leadership on performance improvement plans.
  • Serve as a key operational partner to physician leadership.
  • Provide physician performance reporting and operational insights.
  • Support design and execution of physician compensation and incentive models aligned with: Productivity, Quality outcomes, Value-based care performance.
  • Drive provider engagement, accountability, and alignment with organizational goals.
  • Build and maintain provider schedules across facilities.
  • Align staffing with facility density and growth strategy.
  • Monitor admissions, visits, discharges to ensure target visits for patients are met.
  • Serve as the primary operational liaison to SNF administrators and DONs.
  • Ensure strong onboarding and ongoing engagement at each facility.
  • Proactively address: Service concerns, Coverage issues, Patient assignment challenges.
  • Support growth and retention by maintaining high facility satisfaction.
  • Partner with business development to increase density in existing facilities, add new facilities and expand geographic footprint.
  • Identify opportunities to grow patient volume and strengthen referral networks.
  • Lead, coach, and develop staff.
  • Foster a culture of accountability, teamwork, and continuous improvement.
  • Demonstrate strong leadership capabilities, including: Critical thinking, Conflict management, Negotiation and influence, Staff development and coaching.
  • Partner with HR on recruitment, hiring, and workforce planning.
  • Ensure timely hiring processes and alignment of roles with business needs.
  • Ensure compliance with CMS, regulatory requirements, and organizational policies.
  • Maintain audit readiness and proper documentation standards.
  • Support credentialing processes for physicians and advanced practice providers.
  • Ensure adherence to: Medicare and payer billing requirements, SNF regulatory expectations.
  • Monitor documentation quality and audit readiness.
  • Support implementation of workflows (e.g., TCM, APCM, ACP where applicable).
