The Postural Orthostatic Tachycardia Syndrome (POTS) Clinic RN Coordinator is responsible for performing individualized and age-specific nursing assessment of assigned patients using advanced assessment skills and other medical data to formulate nursing diagnoses. This role involves developing individualized care plans and providing and coordinating comprehensive nursing care. The coordinator instructs patients and significant others in care provided, disease and/or illness, expected outcomes, and home care. The POTS Clinic RN Coordinator supports patients referred for evaluation of POTS and related autonomic conditions, assisting with intake, visit preparation, symptom management, education, and coordination of care during the diagnostic process. Following physician evaluation, the RN plays a key role in care transitions, supporting patients and families with clear care plans, education, and follow-up recommendations and communication.
