The Postural Orthostatic Tachycardia Syndrome (POTS) Clinic RN Coordinator is responsible for performing individualized and age-specific nursing assessment of assigned patients using advanced assessment skills and other medical data to formulate nursing diagnoses. This role involves developing individualized care plans, providing and coordinating comprehensive nursing care, and instructing patients and their significant others on care, illness, expected outcomes, and home care. The Coordinator supports patients referred for POTS and related autonomic conditions, assisting with intake, visit preparation, symptom management, education, and care coordination during the diagnostic process. Following physician evaluation, the RN is crucial in care transitions, providing clear care plans, education, and follow-up recommendations.
