About The Position

The Post Majority Care Worker will be part of Atikameksheng Anishnawbek Family Advocacy Team and will carry out similar functions to a Band Representative as outlined in the Child, Youth and Family Services Act (CYFSA) as it relates to youth in care. The position will provide culturally grounded, holistic, and strengths-based support to Atikameksheng Anishnawbek youth transitioning out of care into adulthood.

Requirements

  • Post-secondary education in Social Work, Child and Youth Care, Indigenous Studies, Psychology, or a related human services field. A diploma with significant relevant experience may be considered.
  • Minimum of 2–3 years of experience working directly with Indigenous youth and/or families, preferably in child welfare, post-majority services, or family advocacy, Band Representative.
  • Experience providing case management, transition planning, or advocacy for youth and young adults.
  • Experience working with Indigenous communities and demonstrated ability to build positive relationships with youth, families, and service providers.

Responsibilities

  • Liaise and collaborate with child welfare agencies to ensure smooth transfer and support of youth transitioning into Post Majority services.
  • Conduct intakes and develop individualized transition plans tailored to each youth’s needs and goals.
  • Provide ongoing case management, including coordination, follow-up, and documentation of services.
  • Support Indigenous youth and young adults in developing, monitoring, and evaluating individualized life plans, including mental health, addictions, and overall well-being.
