The Post Heart Transplant Nurse Coordinator is a professional registered nurse licensed in the state of Massachusetts with cardiac surgery experience. This role is responsible for all aspects of ambulatory and in-hospital care of the pre-transplant and post-transplant patient. The position involves close collaboration with the transplant team, inpatient staff, and others to ensure effective care transitions between pre-, peri-, and post-transplant phases in both ambulatory and hospital settings. Key responsibilities include collecting and entering data into EPIC and heart transplant databases as required by the hospital and various agencies. The role demands a strong clinical background, exceptional organizational skills, attention to detail, and the ability to coordinate and follow through on all aspects of patient care. Effective communication with the transplant team, patients, and families is crucial. The nurse coordinator must demonstrate the ability to effectively manage patients by assuming full responsibility for assessment, planning, implementation, and evaluation of patient care. This position reports directly to a designated nurse manager or nurse director and will work on-site at the BWH Main campus, with potential travel to off-site locations in Massachusetts and surrounding states, possibly requiring licensure in other states.
