About The Position

The Lung Transplant Nurse Coordinator is a professional registered nurse licensed in Massachusetts with transplant experience, responsible for the care of pre-transplant, inpatient, and post-transplant patients. This role primarily focuses on pre-transplant patients but collaborates closely with the entire transplant team to ensure seamless care transitions. Responsibilities include data collection and entry into EPIC and transplant regulatory databases, requiring a strong clinical background, organizational skills, attention to detail, and effective communication. The coordinator must manage patient care independently, demonstrating proficiency in assessment, planning, implementation, and evaluation. The position is based at the BWH Main campus and may require licensure in other states for off-site work.

Requirements

  • Graduate of an approved school of nursing with current registration in Massachusetts required.
  • BSN required.
  • One year or more of prior experience (recent within 2 years) in the care of pulmonary, thoracic surgery or lung transplant patients required.
  • Ability to work independently while collaborating with others in the care of complex patients.
  • Excellent written and oral communication skills.
  • Flexibility, excellent judgment, and the ability to work independently and as part of a team are essential.
  • Excellent organizational abilities to formulate and meet rigorous clinical timetables.
  • Must display interpersonal skills and ability to work as part of a multidisciplinary team.

Nice To Haves

  • Prior experience with Lung Transplant patients strongly preferred.
  • Prior Transplant Coordinator (1 year or more within the last 5 years) experience strongly preferred.
  • Ambulatory experience (current defined as within the last 12 months) with telephone triage (assessment, management, planning, and execution) strongly preferred.
  • Knowledge of community resources and post/non-acute care providers preferred.
  • Familiarity with discharge and resource planning preferred.
  • ABTC Certified Clinical Transplant Coordinator preferred.
  • Active membership in a related professional organization is recommended.
  • May need to obtain necessary nurse licensure in other states.

Responsibilities

  • Reviews referral documentation and medical records to identify potential transplant patients and coordinates pre-transplant evaluations.
  • Collaborates with the transplant team for seamless coordination of newly referred patients.
  • Coordinates and facilitates New Patient and Pre-Transplant clinic sessions, ensuring completion of necessary testing.
  • Participates in the informed consent process, providing patient education on risks, benefits, procedures, and medications.
  • Facilitates the completion of the evaluation for lung transplant candidacy, ensuring multidisciplinary participation and identifying outstanding items.
  • Compiles and summarizes evaluation results for presentation at multidisciplinary team meetings and documents outcomes.
  • Facilitates and coordinates educational programs for patients and families.
  • Collaborates with the Pre-Transplant Administrative Coordinator to facilitate patient care.
  • Works with the financial coordinator to provide medical information for third-party payers.
  • Communicates patient information to the care team via documentation and conferences.
  • Conducts post-acute patient follow-up to ensure execution of care plans and optimal service utilization.
  • Manages systems to provide information on patient care levels and clinical status changes to relevant parties.
  • Works with Physician Directors to review, revise, and initiate improvements to the pre-transplant management process.
  • Participates in multidisciplinary patient care conferences and medical management meetings.
  • Ensures documentation is clear, complete, concise, and organized.
  • Observes principles of infection control in nursing practice.
  • Assists with preparation of reports and data for utilization of services review, QM activities, UNOS, and Center of Excellence contracts.
  • Participates in assigned QAPI processes and e-care improvement initiatives.
  • Seeks ongoing feedback to ensure nursing practice remains at the highest level.
  • Coordinates out-patient care of pre lung transplant patients, including participation in multidisciplinary rounds and documentation.
  • Facilitates interactions of multidisciplinary team members.
  • Acts as a liaison between inpatient and outpatient management teams.
  • Communicates with referring physician teams regarding patient status.
  • Conducts telephone contact with patients to assess clinical status, monitor therapy, adjust medications, and organize follow-up.
  • Assists in urgent patient triage, including ER visits and transfers.
  • Provides education to transplant patients and family members on various aspects of care.
  • Sets up consults with specialty providers, follows up on recommendations, and communicates results.
  • Assists with communication and correspondence to/from patients, employers, pharmacies, and insurance providers.
  • Meets regularly with medical program director and nurse practitioners to facilitate quality and process improvement initiatives.
  • Attends weekly clinical meetings and lung transplant clinics.
  • Works with financial counselor and social work to assist patients/families with support needs.
  • Cross-trains to assist in covering for inpatient and post-heart transplant nurse coordinators.
  • Works on-campus wearing appropriate PPE when caring for patients requiring enhanced precautions.
  • Triage and manages high patient volumes with complex needs.
  • Collects demographic and clinical data for transplant databases, including UNOS.
  • Provides research nurses with clinical data on research study subjects.
  • May assist in writing summaries for administrative or clinical purposes.
  • Establishes effective professional relationships with healthcare team members, patients, families, and referring providers.
  • Serves as a role model and resource for other staff.
  • Maintains communication with other transplant units/programs.
  • Maintains professional relationships with appropriate professional organizations.
  • Regularly incorporates new scientific findings into clinical practice.
  • Maintains awareness of new developments in the specialized area and participates in research activities.
  • Utilizes time and materials effectively.
  • Participates in the orientation of new staff members, students, and others.
  • Provides input into the evaluation of other staff.
  • Adheres to hospital and departmental policies and recommends changes.
