About The Position

The Post-Acute Manager is responsible for overseeing and coordinating clinical and administrative operations supporting post-acute and long-term care facilities. This role ensures accurate patient documentation, efficient provider and facility coordination, regulatory compliance, and high-quality patient care. The Post-Acute Manager works closely with medical providers, nursing facilities, internal operations teams, and revenue cycle management (RCM) to support optimal clinical outcomes and operational efficiency.

Requirements

  • High school diploma or equivalent required
  • CPR certification required
  • Strong communication, organizational, and problem-solving skills.
  • Proficiency with Microsoft Office Suite and electronic medical record (EMR) systems.
  • Demonstrated technical proficiency and comfort using mobile devices (e.g., iPads) for charting and documentation.

Nice To Haves

  • Associate’s degree or higher preferred
  • Registered or Certified Medical Assistant preferred; alternatively, Certified Nurse Aide (CNA) or Certified Medication Aide.
  • Minimum of 3 years of clinical experience in a medical office, post-acute, or nursing facility setting preferred.

Responsibilities

  • Partner with leadership to develop, improve, and expand medical care services within post-acute and long-term care facilities.
  • Ensure the accuracy, completeness, and timeliness of patient records in accordance with company standards and regulatory requirements.
  • Coordinate provider schedules and clinical workflows across nursing and long-term care facilities.
  • Facilitate specialty referrals, imaging requests, medication authorizations, and related follow-up as needed.
  • Implement and support new clinical tools, workflows, and resources in collaboration with the medical care team.
  • Serve as a primary point of communication for operational issues, escalating concerns to the Director of Operations as appropriate.
  • Collaborate with the Revenue Cycle Management (RCM) team to ensure accurate and timely flow of patient and clinical information from care teams to billing.
  • Work directly with long-term care facilities to resolve issues, improve efficiencies, and streamline information exchange.
  • Assist with provider credentialing by gathering, verifying, and submitting required documentation as requested by upper management.
  • Uphold patient and client rights while maintaining strict confidentiality and HIPAA compliance at all times.
  • Investigate, document, and resolve incidents, complaints, and compliance issues in accordance with company policy and regulatory guidelines.
  • Support screening, hiring, onboarding, and disciplinary actions for post-acute coordinators staff as needed.
  • Perform additional duties as assigned to support operational and clinical goals.
