About The Position

Trillium Health Resources has a career opening for a Population Health Specialist III to join our team! The Population Health Specialist III provides behavioral health clinical coordination for members with complex behavioral, social, and psychosocial needs across population health programs. The role applies behavioral health clinical judgment to assess needs, engage members in appropriate services, and coordinate care with multidisciplinary teams to improve outcomes.

Requirements

Master's degree in health and human services or related field and one (1) year of experience in a clinical setting working with the population served.
Active North Carolina licensure as: Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychological Associate (LPA), Licensed Clinical Addictions Specialist (LCAS), or Registered Nurse (RN)
Must have a valid driver’s license.
Must reside within North Carolina.
Must be able to travel within catchment as required.

Nice To Haves

Five (5) years post-Master’s experience in a clinical setting with the BH/MH/SU/IDD population
A minimum of two (2) years Maternal health and Opioid experience.

Responsibilities

Assess member needs, risks, and barriers related to behavioral health and psychosocial factors.
Coordinate services across providers, care teams, and community resources to support member needs.
Apply clinical judgment to support engagement in appropriate behavioral health and population health services.
Monitor member progress and adjust coordination efforts to support improved outcomes.
Facilitate communication among multidisciplinary teams to ensure coordinated care delivery.
