Ensures seamless network integration and daily network operation success. Attends to operational and clinical needs as well as data, quality, and educational needs. Utilizes knowledge of patient's age and cultural diversity into the provision of patient care.

Requirements

  • Bachelor’s Degree Healthcare or 5 years' experience in a healthcare setting will be considered as equivalent to Bachelor's degree.
  • 3 years Office coordinator or project coordinator and/or manager.
  • Ability to multi-task and handle interruption with ease.
  • Demonstrates a functional knowledge of insurance, managed care plans, and medical authorizations.
  • Excellent computer skills and knowledge of EMR clinical systems and data collection.

Responsibilities

  • Functions as a lead team member to support goals and objectives and facilitates new hire orientation and onboarding.
  • Generates reports using data from EMR, program, and hospital databases and assists with quality and process improvement initiatives.
  • Leads the team in the development, implementation, and use of protocols that ensure team effectiveness in improving patient care quality, service, and resource allocation.
  • Ensures accurate data entry and process flow into the EMR and other databases.
  • Arranges orientation speakers, room assignments, and orientation schedule for new team members.
  • Compiles reports on referrals and provides budget input for clinical needs.
  • Assesses and implements the clinical needs of the practice and provides input to performance appraisals.
  • Triages phone calls from network to ensure staff and patient needs are met.
  • Manages and facilitates effective patient transitions across the continuum with ongoing quality improvement monitoring.
