About The Position

The Population Health Medicaid Coordinator is responsible for supporting the organization's population health initiatives by coordinating care for Medicaid patients. This role focuses on improving health outcomes, closing care gaps, supporting preventive services, and assisting patients in navigating the health care system. The coordinator works closely with providers, clinical staff, and the Population Health Program Coordinator to track quality measures and ensure compliance with Medicaid program requirements.

Requirements

  • Nursing degree (LPN or RN) or Bachelor's degree in health administration or health-related field.
  • Minimum 3-4 years of experience in health care setting with minimum one year of experience in quality management, preferably in medical clinic practice setting.
  • Current LPN license in the State of Missouri or Compact Licensure

Responsibilities

  • Identify and manage Medicaid patients who require preventive services, chronic disease management, or follow-up care.
  • Conduct patient outreach via phone, MyChart, or mail to schedule appointments and encourage compliance with care plans.
  • Assist patients with care coordination, including referrals, follow-up appointments, and community resources.
  • Monitor and track quality metrics and care gaps for Medicaid populations.
  • Document all outreach, care coordination, and patient interactions in the electronic health record (EHR).
  • Collaborate with providers and clinical staff to improve patient outcomes and reduce preventable hospitalizations or ER visits.
  • Provide education to patients on preventive care, screenings, and chronic condition management.
  • Assist with reporting population health data to leadership.
