About The Position

Carolina Family Health Centers, Inc. is a nonprofit, federally qualified health center (FQHC) dedicated to providing accessible and affordable healthcare to the community. They offer integrated primary medical care, behavioral health, pharmacy, and dental services. The Population Health Manager - Medicare, Registered Nurse, will oversee the daily operations of the Population Health Program at the Wilson Community Health Center. This role involves supervising case managers who provide transition of care, care coordination, and chronic care management, with a focus on Medicare patients. The manager will also perform annual wellness visits, supervise others performing this task, and manage a small caseload of patients. This position offers a 40-hour workweek, no weekends, full benefits, a bi-annual bonus, 12 paid holidays, and bi-weekly pay.

Requirements

  • Three years in a healthcare environment.
  • One year supervisory/management background.
  • Bachelor of Science in Nursing (RN).
  • Licensed as a Registered Nurse in the state of North Carolina.
  • Active North Carolina Driver License.
  • BLS Certification.

Responsibilities

  • Provides direct supervision to the case managers, including interviewing, hiring, training, orientation, assigning responsibilities, and ongoing performance evaluation.
  • Monitors staff productivity and department success through regular reporting and communication to executive staff.
  • Performs audits and runs reports to monitor staff adherence to CFHC, Inc., Medicare, and other regulatory agencies' rules and regulations for providing and documenting case management services.
  • Attends meetings and participates in committees as assigned.
  • Reviews software system functionality and develops and documents efficient workflows for staff.
  • Serves as a point of contact for local hospitals’ discharge planners to identify patients admitted to the hospital or who have used the emergency room to reduce 30-day hospital readmission rates or inappropriate emergency room use.
  • Performs annual wellness visits under the supervision of the patient’s assigned medical provider.
  • Provides case management/chronic care management services to a small caseload of patients.
