Trillium Health Resources has a career opening for a Population Health Nurse to join our team! The Population Health Nurse provides clinical care management and population health support for assigned members, focusing on prevention, wellness, and longitudinal health outcomes. The role applies nursing judgment as a licensed registered nurse to analyze data, identify care needs, and coordinate services using population health management approaches that improve outcomes and reduce reliance on episodic care.

Requirements

  • Associate’s degree in Nursing and one (1) year of experience as a Registered Nurse.
  • Active North Carolina Licensure as a Registered Nurse (RN).
  • Must have a valid driver’s license.
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.

Nice To Haves

  • Experience working with BH/MH/SU/IDD population.
  • Knowledge of QM and UM procedures, as well as experience in using data analytics for population health management.
  • Certified Managed Care Nurse (CMCN), Certified Case Manager (CCM), Accredited Case Manager (ACM) or equivalent certifications in care management or nursing.

Responsibilities

  • Conduct clinical assessments to identify member health needs, risks, and care gaps.
  • Develop and implement care plans to support preventive care, chronic condition management, and wellness goals.
  • Coordinate services across providers, care teams, and community resources.
  • Monitor member progress and adjust care plans based on clinical indicators and outcomes.
  • Provide health education and coaching to support self-management and engagement.
