About The Position

Are you passionate about improving health outcomes, advancing health equity, and creating meaningful community impact? We are seeking a Population Health Integrations Analyst to lead the development and execution of innovative population health strategies that improve access to care, reduce disparities, and enhance health outcomes across diverse populations. In this role, you will serve as a strategic leader driving the integration of population health principles into care delivery, outreach, education, and care coordination programs. You will collaborate across clinical, quality, operational, and community-based teams to ensure compliance with regulatory requirements while delivering programs that support preventive care, member engagement, and continuity of services. • Strong analytical skills with the ability to interpret performance metrics, identify trends, assess risk, determine root causes, and implement sustainable improvements.
  • Excellent communication and relationship-building skills with the ability to influence stakeholders across clinical, operational, and compliance teams.
  • Demonstrated ability to supervise, mentor, and develop high-performing teams while fostering accountability and continuous improvement.
