About The Position

The Population Health Director will be responsible for the operational aspects of managing various grants and evaluating respective programs. This role involves providing technical assistance to internal and external customers, delivering education services to providers and staff, fostering community collaboration, and preparing reports. The position also includes the management and development of population health activities and Community Health Worker (CHW) staff. Specific responsibilities will be assigned within this general scope.

Requirements

  • Supervisory/managerial experience.
  • Possess a valid driver’s license.
  • Ability to read, write and speak English.
  • Good interpersonal and human relations skills are required.
  • Must possess skills to empathize and care for a variety of patients, including low-income and diverse cultures and treat everyone (internal and external customers) with respect and dignity.
  • Ability to maintain confidentiality at all times in all situations and follow the laws and intent of the most current HIPAA laws and regulations and Canyonlands’ security and privacy policies.

Nice To Haves

  • Bachelor's and/or Master's degree preferred but not required.
  • Experience with health services administration, public administration or related field.
  • A combination of education and experience to meet the minimum requirements will be considered.
  • Three to five years progressive management experience.
  • Bilingual (English/Spanish/Navajo) proficiency preferred.
  • Knowledge of Microsoft programs with the ability to effectively communicate using Outlook.

Responsibilities

  • Develops, implements, and evaluates programs that provide services to the public.
  • Conducts fiscal and program monitoring to ensure compliance with contract and established clinical, fiscal and administrative program standards.
  • Works collaboratively with funding agencies to develop outcome measures and implement quality improvement initiatives.
  • Participates in the development and administration of grant program budget.
  • Develops policies and procedures related to specific grants and programs.
  • Develops and implements clinical education and training as needed.
  • Completes program and fiscal reports to funding sources.
  • Provides technical assistance to direct service providers.
  • Prepares reports and issues requests for proposals as needed.
  • Collaborates with other organizations to promote coordinated planning and utilization of resources.
  • Assures grant/contract and regulatory compliance.
  • Supervise and evaluate employees.
  • Ensure Community Health Workers (CHWs) receive appropriate training, support and supervision.
  • Manage Community Health Workers (CHWs) in a manner that best supports population health and value based contract activities.
  • Oversees that the organization’s staff are screening and referring, as appropriate, all members to resources that meet the individual’s HRSN and BH needs.
  • Develop programs and services that support the management of various patient populations in an effort to improve health outcomes.
  • Ensures that health inequities amongst the organization’s patient population are identified and creates a plan to address them.
  • Interprets guidelines and rules.
  • Prepares and maintains records and reports.
  • Designs, conducts and evaluates community outreach.
  • Provide ongoing evaluation of the quality and effectiveness of education activities to ensure compliance with specific Grant, CCHC Policy, State and Federal requirements.
  • Complies with CHC Standards of Conduct, Code of Conduct and Corporate Compliance.
  • Serves on various CHC and community committees as required/requested.
  • Attends meetings, seminars, and conferences as appropriate to ensure skills and knowledge remains current.
  • Other duties as assigned.
