About The Position

In this role, you will empower individuals and families to overcome barriers to care, navigate complex healthcare systems, and access the resources they need to thrive. Whether supporting a patient through a Medicaid renewal, helping coordinate transportation to an appointment, or connecting someone to food, housing, or behavioral health resources, your work will directly impact lives across our community. Requirements

  • Associate degree in a healthcare-related field or equivalent combination of education and experience
  • Certified Community Health Worker (CHW) or eligible to obtain certification
  • 3+ years of experience in a healthcare or community health environment
  • Knowledge of Medicaid/Medicare enrollment and coverage processes
  • Understanding of Social Determinants of Health (SDOH) and community-based care
  • Strong communication and relationship-building skills
  • Experience working with diverse populations and interdisciplinary teams
  • Comfort using laptops, tablets, and electronic health record systems

Nice To Haves

  • Missouri CHW Certification through Metropolitan Community College
  • Leadership or mentoring experience
  • Experience documenting within Electronic Health Records (EHR)
  • Knowledge of chronic disease management and community health impacts
  • Bilingual abilities are highly valued

Responsibilities

  • Conduct Social Determinants of Health (SDOH) assessments and identify barriers to care
  • Engage high-risk patients through outreach, education, and relationship-building
  • Develop and support individualized care support plans
  • Assist patients with appointment scheduling, follow-up care, and referral coordination
  • Support smooth transitions of care following inpatient or emergency department visits
  • Coach and educate patients managing chronic conditions
  • Encourage medication adherence and care plan compliance
  • Help resolve transportation, communication, and access challenges
  • Document patient interactions and track progress toward health and wellness goals
  • Guide patients through Medicaid enrollment and renewal processes
  • Help prevent gaps in healthcare coverage through proactive outreach
  • Connect patients with educational resources related to insurance changes and eligibility
  • Collaborate with Patient Access and clinical teams to ensure continuity of care
  • Connect patients with internal and community-based resources that improve stability and well-being
  • Advocate for patients and help strengthen patient-provider relationships
  • Support efforts to reduce appointment no-show rates and improve engagement
  • Serve as a mentor, trainer, and educator when needed
  • Support Community Health Workers and Population Health initiatives
  • Promote quality improvement, cultural diversity, patient safety, and organizational excellence
  • Represent the mission and values of University Health KC throughout the community
