The population health coordinator will perform comprehensive tracking and follow-up for the purpose of achieving outreach program goals. Maintain records, statistics and databases. Utilizes EMR generated reports to identify and engage in patient outreach. Requires the ability to work as a team and contribute to the development of an effective work environment.

Requirements

Two or more years’ experience in clinical or community health setting required.
Must have good customer service and phone etiquette.
Must have proficient computer skills, Excel, Word, and Data entry.
Medical Assistant certification, or current California Licensed Vocational Nurse (LVN) license required.

Responsibilities

Utilizes exceptional customer service, communication, phone etiquette and critical thinking skills
Assisting the coordinating of patient appointments for quality measures
Track patient populations to obtain statistical information related to specific program goals
Uses the EMR system to actively engage in patient outreach.
Send Letters forms and other media required to educate patient population
Must have proficient computer skills including data entry and excel.
Perform other related duties as may be assigned.
