About The Position

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking a Platform Coordinator to join their award-winning team. This role supports Care Coordinators by processing patient referrals from various platforms and ensuring accurate patient information is maintained. The ideal candidate is detail-oriented, a multitasker, and committed to providing high-quality, patient-centered care in a home health setting.

Requirements

  • GED or High School Diploma required; Associate's or Bachelor’s Degree in a related field preferred.
  • Ability to manage conflict, stress, and multiple simultaneous work demands in an effective, professional, positive manner.
  • Ability to analyze and reconcile complex issues and problems timely.
  • Ability to work constructively when faced with high workloads and deadlines.
  • Ability to follow directions and respectfully request clarification when needed.
  • Ability to identify needs and opportunities and present them to the supervisor with recommended solutions.
  • Ability to work independently with minimal supervision while collaborating with other team members.
  • Ability to build and maintain relationships.

  • Working knowledge of EMR database: Allscripts, Navi, Epic NCH, PowerChart.
  • Proficient with Microsoft Word, Excel, and Outlook software.
  • Possesses good customer relations skills and a commitment to providing quality service.
  • Must possess excellent prioritization and time management skills.
  • Must be able to communicate effectively and clearly.

Responsibilities

  • Process patient referrals from Navi, Allscripts, Epic NCH, and Power chart, as well as other referral platforms.
  • Pull referrals off platforms to accept/decline.
  • Review documents, confirm, and process the referral through Intake using Pinnacle’s EMR.
  • Respond to Care Coordinator inquiries regarding referral status promptly.
  • Respond to all inquiries with a resolution to concerns and/or discrepancies.
  • Coordinate and communicate with the appropriate team regarding any changes or updates to the patient’s payer source on time.
  • Meet productivity goals as assigned by the direct supervisor.
  • Comply with HIPAA regulations on all accounts.
  • Perform other responsibilities assigned by leadership.
