About The Position

Reporting to the Planning & Transformation Director, the Planning Manager provides support across sub-regional planning for an assigned health system, driving the execution of planning frameworks and implementation strategies across a complex landscape of clinical, behavioral health, and community-based partners. This position is dedicated to the planning and implementation projects led by Care Compass, through the NYS DOH Rural Health Transformation Program (RHTP). The Planning Manager brings knowledge and experience with health system structure, transformational change, regional partner relationships, and alignment of cross-sector stakeholders. This role ensures that planning and transformation work is regionally coordinated, supports integration across rural health care providers, and aims to build sustainable progress across the region.

Requirements

  • Bachelor's degree in Public Health, Health Administration, Business Administration, or a related field; Master's degree preferred.
  • Minimum of 4–5 years of experience in health care, community health, or program coordination, preferably in a rural health or regional network setting.
  • Demonstrated experience in stakeholder engagement, partner management, and facilitation of multi-organizational workgroups.
  • Strong organizational and project management skills with the ability to manage multiple deliverables and deadlines across sub-regions.
  • Experience coordinating data collection activities and working with diverse data sources including public health, hospital, and social care data.
  • Excellent written and verbal communication skills; ability to translate complex program requirements for diverse audiences.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Teams); familiarity with project management tools preferred.
  • Ability to travel within the assigned sub-region for partner and network meetings.

Responsibilities

  • Serves as the primary Care Compass relationship manager and point of coordination for assigned health system(s), developing trusted relationships with executive, clinical, IT/data, workforce, finance, and other key stakeholders to support RHTP planning and implementation.
  • Develops a comprehensive understanding of assigned health system priorities, organizational structure, capabilities, competing priorities, challenges, existing initiatives, and key personnel to ensure Care Compass planning activities are informed by local needs and existing resources.
  • Coordinates RHTP planning activities between assigned health system(s), Care Compass, external partners and the RHTP Regional Planning Council, and other participating organizations by organizing meetings, facilitating discussions, gathering input, and translating partner feedback into clearly defined planning needs and next steps.
  • Serves as an internal quarterback for assigned health system(s) and partners, connecting partners with appropriate Care Compass subject matter experts and ensuring information related to workforce, technology, data, care delivery, finance, community services, and other initiatives reaches the appropriate teams.
  • Supports development and implementation of regional initiatives by coordinating partner participation, ensuring appropriate stakeholders have been identified and engaged, and supporting resolution of implementation barriers with the implementation team(s).
  • Maintains consistent two-way communication between Care Compass, assigned Health System(s) and other partners, ensuring priorities, decisions, concerns, opportunities, program updates, deadlines, or changing priorities are accurately communicated, documented, and escalated when appropriate.
  • Identifies opportunities where health system priorities may benefit from regional collaboration, shared infrastructure, community partnerships, or coordinated solutions and brings those opportunities forward to the Director and appropriate Care Compass teams for evaluation.
