About The Position
Mozaic (A Chapter of Arc NY) is seeking a compassionate and organized Planning Coordinator to support people receiving services within our Day Services, Day Habilitation, and Community Habilitation programs throughout Yates and Seneca Counties. In this role, you will help ensure people we support receive high-quality, person-centered services that promote independence, community inclusion, and achievement of their personal goals.
Requirements
- Strong organizational skills, attention to detail, and the ability to manage multiple priorities independently.
- Professionalism, strong communication and teamwork skills, sound judgment, and discretion when handling confidential information.
- Proficiency with computers and office technology.
- A valid New York State Driver's License.
- Ability to operate agency vehicles and wheelchair-accessible vans.
- A working telephone must be maintained, with updated contact information provided to the agency.
- High School diploma or equivalent required.
- 1 year of experience working with people with developmental disabilities.
Responsibilities
- Ensuring regulatory and agency compliance while supporting people through person-centered planning and service coordination.
- Monitoring and maintaining Life Plans.
- Ensuring accurate and timely documentation.
- Overseeing program budgets and individual funds in accordance with agency procedures.
- Advocating for the people supported.
- Promoting community integration.
- Developing positive relationships with families, service providers, employers, and community partners while upholding Mozaic's mission, vision, and values.
- Safeguarding the privacy, dignity, and confidentiality of people receiving services.
