About The Position

Mozaic is searching for a Planning Coordinator for Day Services/Community Habilitation in Seneca County to join their growing team of professionals. The Planning Coordinator role ensures regulatory and corporate compliance by adhering to regulations, agency policies, and maintaining accurate documentation. It also involves financial management responsibilities, such as overseeing program budgets and managing individuals' funds in accordance with procedures. The role is centered on providing person-centered services, ensuring individuals' Life Plans and other safeguards are followed, advocating for them, and supporting community integration. It requires fostering positive relationships with internal and external contacts, including families, service providers, and local businesses, while upholding the agency's mission, vision, and values, and respecting individuals' privacy and confidentiality.

Requirements

  • High School diploma or equivalent required.
  • 1 year of experience working with people with developmental disabilities.
  • High level of conscientiousness, dedication, and responsibility.
  • Ability to work independently, prioritize tasks, and manage multiple assignments in a high-pressure environment.
  • Strong teamwork, communication, and discretion in handling confidential information.
  • Proficiency in computer skills.
  • Ability to operate office and medical equipment.
  • Valid NYS driver's license.
  • Ability to operate wheelchair vans.
  • Must maintain a working telephone with updated contact information provided to the agency.
