About The Position

Supports adults with disabilities in achieving their vocational goals by developing person centered plans along with coordinating, implementing and facilitating activities to include, managing the development, maintenance and communication of mandatory tracking and reporting of progress to goals. Serves as a liaison to community partnerships.

Requirements

• High school diploma plus three years of experience of working with adults with developmental and/or cognitive disabilities
• Associate’s degree in human services, education, psychology or related field plus one year of experience of working with adults with developmental and/or cognitive disabilities
• Ability to be positive and outgoing and demonstrate strong interpersonal skills
• Strong organizational skills

Responsibilities

• Developing person centered plans
• Coordinating, implementing and facilitating activities
• Managing the development, maintenance and communication of mandatory tracking and reporting of progress to goals
• Serving as a liaison to community partnerships
• Assisting the people we support to pursue and obtain their personal goals
• Effectively communicating orally, in writing, and on the telephone
• Working effectively without direct supervision to ensure completion of mandatory daily and monthly documentation
• Working cooperatively with all departments, levels of staff, and outside contacts
• Working in varying climate/element/locations (sites are based in the community and may include both indoor and outdoor activities)
• Effectively balancing multiple tasks and ever-changing priorities
• Assisting the People We Support (PWS) with ADL’s (Activities of Daily living) and ILS (Independent Living Skills)
