About The Position

Transitional Services for New York, Inc., (TSINY) not-for profit, is a comprehensive, community-based mental health organization located in New York City. We provide a continuum of rehabilitative services to enrich the lives of those recovering from mental illness and facilitate their transition to increased levels of independence. Transitional Services for New York, Inc. envisions broadening its rehabilitative services and becoming a regional social service provider. Transitional Services for New York Staff will deliver effective programs with compassion, integrity, and professionalism. We expect all staff to put our clients’ needs first while respecting ourselves and each other as we provide hope to those who participate in our programs. TSINY is looking for Peer Counselor for Supported Housing, one of our mental health residential programs located in Queens, NY.

Requirements

High School diploma
one year’s experience in a mental health setting preferred

Responsibilities

Coordinate referral process (obtain documentation, follow-up with referring agent). Upon receipt of a referral, an initial intake appointment should be scheduled within one week.
Enter screening assessments into AWARDS.
Document daily progress and weekly contacts in AWARDS.
Coordinate and process financial assessment forms.
Provide Financial Specialist with any additional financial information/documentation required in a timely fashion.
Obtain HRA 2010e status.
Maintain appropriate and adequate charts on those individuals being referred to TSINY.
Coordinate and prepare documentation for new residents admitted.
Contact appropriate Program Director for the purpose of scheduling orientation meetings, i.e., tours.
Schedule and conduct outreach.
Assist referral agents and other professionals with the preparation of referral data when necessary.
Handle correspondence and telephone inquiries related to intake and/or discharge.
Assist residential staff with discharge planning.
Perform other related duties as required.
