About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The physician will conduct comprehensive health evaluations, including medication and medical history reviews, physical evaluations, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The physician will be part of a national network dedicated to improving health outcomes and connecting millions of people to the right care. The primary workplace is the patient's private residence, which may present diverse conditions including household pets, secondhand smoke, and airborne allergens. The role requires flexibility to travel locally and within licensed states, and may involve conducting virtual visits.

Requirements

Active, unrestricted license(s) in coverage area(s).
Able to cover the following primary locations: Portland and surrounding area.

Responsibilities

Provide In-Home Health Evaluations.
Conduct medication and medical history reviews.
Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
