About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The physician will conduct comprehensive and personalized health evaluations, including medication and medical history review, physical evaluations, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The physician will be part of a national network dedicated to improving health outcomes and connecting people to the right care.

Requirements

Active, unrestricted license(s) in coverage area(s).
Able to cover the primary location: Augusta, ME.

Responsibilities

Conduct In-Home Health Evaluations.
Engage with patients in the comfort of their own homes.
Bridge gaps in care and build trusted relationships.
Conduct medication and medical history reviews.
Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
