About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care, build trusted relationships, and provide flexible, comprehensive, and personalized health evaluations. The physician will conduct approximately 45-minute visits, including medication and medical history reviews, physical evaluations, and potentially diagnostic tests. They will utilize an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The role is part of a national network of clinicians dedicated to improving health outcomes and connecting people to the right care.

Responsibilities

Provide In-Home Health Evaluations.
Engage with people in the comfort of their own homes.
Bridge gaps in care.
Build trusted relationships to make people healthier.
Conduct one-on-one visits to answer health questions.
Perform medication and medical history reviews.
Conduct physical evaluations.
Perform additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
