In-Home Health - Physician (Part Time)- Hartford, CT
About The Position
Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The primary workplace is the patient's private residence, which can vary in conditions. Candidates must be comfortable working in diverse home environments, potentially including household pets, secondhand smoke, and airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Able to cover the following primary locations: Hartford and surrounding area.

Responsibilities
- Conduct In-Home Health Evaluations, engaging with people in the comfort of their own homes.
- Bridge gaps in care and help build trusted relationships.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
- Be flexible to travel locally and within licensed states, as needed.
