About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Physician to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships. The goal is to bring flexible, comprehensive, and personalized health evaluations directly to individuals. The visits, lasting about 45 minutes, allow for one-on-one connection, medication and medical history review, and physical evaluations. Additional diagnostic tests may be performed if ordered by the health plan. Physicians will use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may present various conditions, including household pets, secondhand smoke, and airborne allergens. Candidates must be able to perform duties comfortably and safely in these environments.

Active, unrestricted license(s) in coverage area(s)
Able to cover the following primary locations: Hartford and surrounding area.

Responsibilities

Provide In-Home Health Evaluations.
