About The Position

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The physician will conduct comprehensive and personalized health evaluations, including medication and medical history review, physical evaluations, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and other airborne allergens.

Requirements

• Active, unrestricted license(s) in coverage area(s) OLYMPIA THURSTON.
• Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
• MD or DO degree.

Responsibilities

• Conduct In-Home Health Evaluations.
• Connect one-on-one with individuals during approximately 45-minute visits.
• Review medication and medical history.
• Perform physical evaluations.
• Conduct additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
