About The Position

Signify Health, part of CVS Health®, is seeking a Per Diem Physician to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. The evaluations, lasting about 45 minutes, include medication and medical history review, a physical evaluation, and potentially diagnostic tests. Physicians will use an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The position is part of a national network of clinicians focused on improving health outcomes and connecting people to the right care.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Ability to comfortably and safely perform duties in diverse home environments, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
• Must be able to travel locally and within licensed states, as needed.
• Must be able to conduct virtual visits as needed.

Nice To Haves

• Multi-state licenses are preferred.
• Ability to cover Staunton and surrounding areas.

Responsibilities

• Conduct In-Home Health Evaluations.
• Engage with individuals in the comfort of their own homes.

• Conduct physical evaluations.
• Perform additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Provide virtual visits as needed.
• Fulfill the requirements of the specified role within Signify Health.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
