About The Position

Humana’s Primary Care Organization is a senior-focused, value-based care provider operating under the CenterWell and Conviva brands. The Primary Care Physician at CenterWell Senior Primary Care delivers comprehensive primary care to seniors as part of a collaborative interdisciplinary team that includes advanced practice providers, pharmacists, care coaches, and behavioral health specialists. The position reports to the Associate Medical Director and is focused entirely on clinical work, including direct patient care and general administrative time. The role is performed on-site in an outpatient office setting. Physicians document care in Athena EMR and participate in a limited on-call rotation. The role is patient-facing and requires TB screening if selected. The position is scheduled for 40 hours per week.

Requirements

Graduate of an accredited MD or DO program from an accredited university.
Board certified or board eligible through ABMS or AOA in Family Medicine, Internal Medicine, or Geriatric Medicine.
Current and unrestricted medical license, or willingness to obtain a medical license in the state of practice.
Demonstrated strong interpersonal relationship and communication skills with colleagues and patients.

Nice To Haves

Active and unrestricted DEA license.
Medicare and Medicaid Provider Number.
At least 2 years of experience.
At least 2 years of experience with Medicare Advantage patients.
Knowledge of Medicare standards.
Knowledge of HEDIS quality indicators.

Responsibilities

Provide comprehensive primary care to seniors.
Collaborate daily with the interdisciplinary team.
Document care in Athena EMR.
Participate in a limited on-call rotation.
Spend all working time on clinical responsibilities, including patient-facing care and general administrative time.
