About The Position

Clinica Sierra Vista is seeking a Physician Rheumatologist to provide specialized care in diagnosing and managing patients with rheumatic diseases and autoimmune disorders. This role involves collaboration with licensed independent providers, clinical pharmacists, and care teams within a Federally Qualified Health Center that has a 47-year history of growth and innovation. Clinica Sierra Vista serves patients at 31 health centers in Fresno and Kern counties, offering a wide range of services including Pediatric Care, Adult Care, Dental Care, Behavioral Health, and more. Requirements

• Provide specialized care in diagnosing and managing patients with rheumatic diseases and autoimmune disorders, working in collaboration with our licensed independent providers, clinical pharmacists, and care teams.

Responsibilities

• Provide outpatient consultation and management of patients with rheumatic and autoimmune conditions (e.g., rheumatoid arthritis, lupus, gout, vasculitis, osteoporosis).
• Order and interpret diagnostic tests, including imaging and laboratory studies, to guide treatment decisions.
• Perform joint aspirations, injections, and other rheumatologic procedures as appropriate.
• Develop and implement individualized treatment plans in collaboration with the care team.
• Refer patients to other specialists as clinically indicated.
• Collaborate with primary care providers, clinical pharmacists, and ancillary staff to coordinate comprehensive care.
• Educate patients and families on disease management, medications, and lifestyle modifications.
• Maintain accurate and timely medical records in compliance with CSV policies and federal regulations.
• Participate in quality improvement initiatives and continuing medical education activities.
