About The Position

ChenMed is a physician-led, mission-driven primary care organization focused on providing affordable, high-quality care to seniors. The Primary Care Physician (PCP) provides care for older adults with acute and chronic illnesses, takes accountability for patient outcomes, and works to build trusting relationships that support patients in improving their health. PCPs serve panels of up to 450 patients and are measured on outcomes including admissions, appointment volume, patient experience, clinical gap closure, and medical costs. They work toward established goals with center and market leadership and care teams, and help foster a positive culture of love, accountability, and passion. The PCP works independently and collaboratively with other healthcare professionals, managers, and medical directors. They follow organizational guidelines and participate in quality and risk management programs, clinical meetings, and other activities that promote patient health and company goals. The position requires adherence to departmental objectives, performance standards, regulatory requirements, quality standards, and company policies. The PCP leads a care team for patients able to visit the office and collaborates with transitional care teams and other resources for patients who are hospitalized, in skilled nursing or long-term care, or homebound. They participate in center management, provide coverage for other providers when needed, and may help recruit patients and providers. The role also includes supervising or collaborating with an Advanced Practice Practitioner and remaining accountable for that practitioner’s actions. ChenMed describes itself as family-owned and physician-led, with a focus on improving healthcare for seniors. The organization offers compensation, comprehensive benefits, and career development and advancement opportunities. The posted pay range is $0.00–$0.00 per hour; final compensation may depend on experience, education, location, and other factors, and the position may be eligible for bonuses or commissions.
