Responsibilities
- Perform comprehensive patient evaluations, including history, physical examination, and diagnostic interpretation.
- Diagnose and manage acute, chronic, and complex medical conditions.
- Develop individualized treatment plans and monitor patient progress.
- Order, interpret, and follow up on diagnostic tests (labs, imaging, PFTs, cognitive screenings, cardiac assessments, etc.).
- Provide preventive care, including immunizations, screenings, and lifestyle counseling.
- Manage medication therapy, including prescribing, monitoring, and reconciliation.
- Participate in care gap closure, chronic disease management, and preventive health initiatives.
- Utilize evidence‑based guidelines to improve patient outcomes and reduce avoidable hospitalizations.
- Document care accurately to support quality reporting, risk adjustment, and compliance.
- Engage in population health strategies, including outreach, follow‑up, and care coordination.
