Requirements

  • Perform comprehensive patient evaluations, including history, physical examination, and diagnostic interpretation.
  • Diagnose and manage acute, chronic, and complex medical conditions.
  • Develop individualized treatment plans and monitor patient progress.
  • Order, interpret, and follow up on diagnostic tests (labs, imaging, PFTs, cognitive screenings, cardiac assessments, etc.).
  • Provide preventive care, including immunizations, screenings, and lifestyle counseling.
  • Manage medication therapy, including prescribing, monitoring, and reconciliation.
  • Participate in care gap closure, chronic disease management, and preventive health initiatives.
  • Utilize evidence‑based guidelines to improve patient outcomes and reduce avoidable hospitalizations.
  • Document care accurately to support quality reporting, risk adjustment, and compliance.
  • Engage in population health strategies, including outreach, follow‑up, and care coordination.
