Requirements

• Board certification in Family Medicine is mandatory.
• Must possess an active Michigan medical license prior to name clearance.
• Prior experience in Family Medicine is essential.
• Candidates should be located within Michigan.
• A self-query from the National Practitioner Data Bank (NPDB) is required, to be conducted within the past month.
