About The Position

Sutter East Bay Medical Group (SEBMG) is recruiting for a Board Eligible or Board-Certified Ambulatory Palliative Care Physician. Provide Palliative Medicine consultation services and participation in Palliative Supportive Care Services interdisciplinary team. Founded in 2005, Sutter East Bay Medical Group (SEBMG) is a rapidly growing multi-specialty medical group whose physicians and advanced practice clinicians work in more than ten care centers and four hospitals in the San Francisco East Bay Area. Sutter East Bay Medical Group is affiliated with the Sutter East Bay Medical Foundation (SEBMF), a not-for-profit health care organization providing operational and administrative support to our medical group. The foundation provides the latest technology and clinical support, allowing clinicians to focus on delivering exceptional patient care. SEBMG includes 400+ clinicians in more than twenty specialties and is dedicated to providing exemplary health care to the communities we serve. Provide Palliative Medicine consultation services in multiple settings: Home, SNF, RCFE, Hospital and Office (Palliative Care, Cancer Center, and embedded primary care clinics). Coordinate with interdisciplinary team members - each team has an NP, licensed clinical social worker, RN, chaplain, and care coordinator. Facilitate access to appropriate supportive care services. Coordinate care with inpatient palliative care colleagues, referring providers, outside agencies and local hospices. Clear referral criteria (i.e., no chronic pain management). Assist in the development and growth of our outpatient program by means of quality improvement projects and educational outreach.

Requirements

  • Fellowship in Hospice and Palliative Care Medicine
  • Board-certified or board-eligible in Hospice and Palliative Care Medicine
  • CA physician license or eligible for CA physician license
