About The Position

Sutter East Bay Medical Group is seeking a Palliative Care Advance Practice Clinician for our Oakland office. This role involves joining a community-based palliative care team that reaches patients in various settings including clinic, home, assisted living, nursing facilities, and via telemedicine. The position requires collaboration with an integrated, multidisciplinary palliative care team, including MD, APP, RN, SW, chaplain, and CM support. The clinician will work with colleagues within SEBMG to guide treatment plans aligned with patient/family values and participate in an embedded oncology practice at the new Stanford Medicine Sutter Health Cancer Center (SMSHCC). The role offers excellent work/life balance with regular hours, and no night shifts or weekend call.

Requirements

For PA: PA certificate in Palliative and Serious Illness Care
Current NCCPA certification
12+ months of post-graduate clinical experience in hospice, palliative care, oncology and/or geriatrics

Nice To Haves

For NP: credentialed as ACHPN (Advanced Certified Hospice and Palliative Nurse)
Current APRN license
