The Medical Director/Supervising Physician provides clinical leadership and physician oversight for the interdisciplinary care team as part of the Care at Home Solutions program. The Medical Director partners closely with the Advanced Practice Provider (APP), Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Pharmacist, Community Health Worker (CHW), and Care Navigators to ensure delivery of high-quality, evidence-based, patient-centered care for individuals with complex medical, behavioral, and social needs. The Medical Director provides physician supervision and consultation, supports clinical decision-making and care planning, oversees quality and patient safety initiatives, and collaborates with primary care providers to improve health outcomes, reduce avoidable utilization, and achieve performance goals within value-based care programs. The Medical Director is responsible for managing four core pillars of the program: Annual Wellness Visits (AWVs), Transitions of Care (TOC), Complex Care Management (CCM), and Disease Management.
