About The Position

This position provides direct primary medical care to all participants of the program. The role involves performing comprehensive history and physicals, evaluating and treating participants with episodic illnesses, and managing the care of participants in the nursing home. The physician will also admit participants to the hospital, providing primary care responsibilities and coordinating discharge planning. Active participation in family conferences, team meetings, committees, and work groups is expected.

Requirements

• Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) from an accredited school required.
• At least 2-3 years’ experience, minimum one year experience working with a frail/elderly population.
• Current license to practice medicine in SC.
• BLS Required.
• Proven clinical leadership.
• Ability to get along with others in a team environment.

Responsibilities

• Performs comprehensive history and physical on new referrals.
• Performs interval history and physicals on established participants completed quarterly or as indicated.
• Evaluates and treats participants with episodic illnesses.
• Refers participants to medical specialists as indicated.
• Admits participants to the hospital, providing primary care (attending) responsibilities for management, providing updates to the team, judiciously utilizing specialty consultants, and coordinating discharge planning with the team.
• Manages all care of participants in the nursing home, providing regular visits, performing telephone contacts with nursing home staff as required, and admitting nursing home participants to the hospital when necessary.
• Actively participates in family conferences.
• Participates in team meetings, committees and work groups as assigned.
