Physician, Geriatrician (Santa Rosa, CA)
About The Position
Sutter Medical Group of the Redwoods (SMGR) is recruiting a Board-Certified Geriatrician to join our established Geriatric practice within Sutter Senior Care. This is an exciting opportunity to practice in a team-based, value-driven outpatient setting focused on delivering high-quality care to older adults, particularly those with complex medical and social needs. The ideal candidate will have experience in a geriatric group practice or multi-specialty outpatient setting, with a strong background in primary care geriatrics and interdisciplinary team-based care. This position is an outpatient-only setting with a Monday through Friday clinic schedule and shared call with a physician and APC team. There is an emphasis on value-based, panel-oriented care.
Requirements
- MD or DO; Board Certified in Geriatrics, Internal Medicine, or Family Medicine
- Valid and unrestricted California medical license (or eligibility)
- Strong clinical, communication, and patient engagement skills
- Passion for caring for older adults and underserved populations
Nice To Haves
- Experience in a geriatric group practice or multi-specialty outpatient setting
- Experience in outpatient geriatrics or primary care
- Experience in interdisciplinary, team-based care models
Responsibilities
- Provide comprehensive outpatient care for geriatric patients
- Diagnose and manage chronic and complex conditions (e.g., diabetes, hypertension, heart disease, cognitive disorders)
- Deliver preventive care, screenings, and immunizations tailored to seniors
- Manage polypharmacy and medication safety
- Collaborate with APCs, nurses, social workers, and specialists to deliver coordinated care
- Coordinate referrals and support care transitions across settings
- Educate patients and caregivers on disease management and healthy aging
- Participate in team huddles, quality initiatives, and value-based care programs
