About The Position

Requirements

• Board Eligible or Board Certified in General Neurology.
• Must be eligible to work in the United States.
• Must be able to obtain Illinois medical license.
• Must be able to obtain DEA registration.
• Must be able to obtain NPI number.
• Must be able to obtain Medicare/Medicaid enrollment.
• Must be able to obtain hospital privileges.
• Must be able to obtain all other required licenses and certifications.

Responsibilities

• Provide outpatient clinic services in Crystal Lake, IL.
