About The Position

HealthOne Medical & Wellness is seeking Family Physicians to join their growing multidisciplinary health and wellness clinic. This role offers the opportunity to build a long-term family practice with strong clinic support and contribute to the development of a new FHO+ model. Initially, the practice will focus on walk-in medicine, transitioning to a greater emphasis on longitudinal care as the FHO+ model evolves. The physician will work closely with a multidisciplinary team to provide patient-centered care.

Requirements

• Patient focused and service minded approach to delivering excellent, comprehensive care.
• Empathetic, collaborative approach and ability to work effectively with a wide range of internal and external stakeholders.
• Commit to working at HealthOne for a minimum 4 days per week + 1 weekend day per month, with reliable scheduling.
• Licensed to practice as a Family Physician in Ontario and in good standing with the CPSO.

Nice To Haves

• 1+ year of experience preferred, however new grads are also welcomed.

Responsibilities

• Build strong relationships with patients and provide an exceptional care experience, with a focus on compassion and clinical excellence.
• Support patients with their acute care, preventative care, and chronic disease management needs with clear education, shared decision making, and appropriate continuity.
• Apply full scope of practice to create thorough diagnoses and treatment plans, utilizing the multidisciplinary team to support patients with all interconnected aspects of their health journey.
• Engage with the multidisciplinary team and internal stakeholders through on-site education sessions and care innovation initiatives across HealthOne.
