Requirements

Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited school of medicine
Completion of an accredited Residency Program
Licensed or eligible for licensure in Indiana
Board-Certified or Board-Eligible
Active and unrestricted DEA license or ability to obtain prior to start
Commitment to the mission of Indiana Health Centers, Inc.

Responsibilities

Provide primary medical care to community-based patient populations.
Collaborate with integrated behavioral health services.
Participate in a team-based collaborative care model.
