About The Position

ProHealth Care is looking for a dedicated physician to join their team at the Oconomowoc clinic, part of ProHealth Medical Group (PHMG). In this role, the physician will provide comprehensive care to Family Medicine (FM) patients, ensuring a continuum of care for individuals and families within the community. The Oconomowoc Clinic is a state-of-the-art facility dedicated to providing high-quality, patient-centered care. Requirements

• Board-certified or board-eligible in Family Medicine.
• Active Wisconsin medical license (or ability to obtain).
• Strong commitment to providing patient-centered care within a team-based setting.
• Committed to serving a diverse patient population, including families, seniors, and individuals with complex medical needs.
• Collaborative and eager to work within a multidisciplinary team.

Responsibilities

• Provide primary care services for Family Medicine patients, from routine check-ups and preventive care to complex diagnoses and management of chronic conditions.
• Develop and maintain long-term relationships with patients and families, offering continuity of care and supporting their overall health journey.
• Work collaboratively with a multidisciplinary team to ensure the highest standard of care and optimal patient outcomes.
• Participate in clinic meetings, contribute to quality improvement initiatives, and stay informed about the latest medical advancements.
