About The Position

This is an outpatient primary care role focused on full-scope Family Medicine services, offering the opportunity to build meaningful long-term patient relationships within underserved communities. The position features a structured weekday schedule with dedicated administrative time for documentation and follow-up care, a collaborative team-based care model with established workflows and EMR systems, and is located in a historic South Jersey riverfront community with a small-town atmosphere and convenient Philadelphia access.

Requirements

• Board Certified Physician Assistant REQUIRED
• Clear and active or eligible New Jersey PA license REQUIRED
• Comfortable managing full-scope outpatient primary care REQUIRED
• Strong patient-centered communication skills REQUIRED
• Ability to work independently within collaborative team environment REQUIRED
• Passion for serving underserved and diverse communities REQUIRED

• Bilingual Spanish preferred

Responsibilities

• Provide comprehensive outpatient primary care services across the lifespan
• Manage acute and chronic medical conditions
• Coordinate specialty referrals and follow-up care
• Deliver patient-centered treatment plans and documentation
• Collaborate with interdisciplinary support teams and providers
• Support continuity of care within underserved patient populations
