About The Position

The Centre for Family Medicine (CFFM) in Kitchener, Ontario is seeking a Physician Assistant (PA) to join their collaborative primary care team. This role offers the opportunity to work closely with a family physician in a supportive, interdisciplinary environment while making a meaningful difference in the health of patients and families across the lifespan. CFFM is one of two Family Health Teams in the Kitchener-Waterloo region and is known for its strong culture of teamwork, innovation, and patient-centred care. Their interdisciplinary team includes physicians, nurse practitioners, nurses, pharmacists, social workers, and other allied health professionals working together to provide comprehensive care to their community. They are looking for a dynamic, confident, and collaborative PA who enjoys the breadth of family medicine and values building long-term relationships with patients.

Requirements

  • Graduate of an accredited Physician Assistant program
  • Eligible for or successfully completed the Canadian Physician Assistant Certification Exam (PACCC)
  • Eligible for membership with the Canadian Association of Physician Assistants (CAPA) and/or provincial regulatory requirements
  • Eligible for membership with the Canadian Physicians & Surgeons of Ontario (CPSO) Physician Assistant Stream and/or provincial regulatory requirements
  • Strong clinical assessment and diagnostic reasoning skills
  • Demonstrated commitment to evidence-based primary care
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively within an interdisciplinary team
  • Strong organizational and problem-solving skills
  • Commitment to patient-centred care and service excellence

Responsibilities

  • Work closely with a family physician to provide comprehensive primary care services in a busy and rewarding clinical practice.
  • Provide direct patient care.
  • Follow-up of investigations.
  • Provide virtual care.
  • Support patients with medical documentation and care coordination.
  • Conduct telephone or virtual appointments to review results, follow up on investigations, and address patient concerns.
  • Complete medical forms and documentation (insurance forms, disability forms, school/work forms), often including brief follow-up calls with patients when required.
  • Order and interpret diagnostic investigations.
  • Develop and implement care plans in collaboration with the supervising physician.
  • Provide health promotion and preventive care counselling.
  • Coordinate care with specialists and allied health professionals.
  • Support care in local nursing homes, including patient assessments and follow-up as needed.
  • Perform procedures within scope and under medical directives or physician delegation.
  • Maintain clear, accurate clinical documentation in the EMR.
  • Contribute to a collaborative, interdisciplinary care environment.
  • Support the education of learners (e.g., medical students, residents, or PA students) when applicable.
