About The Position
We invite Physician Assistants and Nurse Practitioners to discover what’s possible when healing hearts, inquisitive minds, and visionaries come together in our Adult Medical Specialty team within the Post Acute Network — Skilled Nursing Facility (SNF) program in Charlotte, NC. Join our team-oriented group of compassionate clinicians in North Charlotte/ Cabarrus area, where supportive leadership, strong work-life balance, and the opportunity to deliver exceptional patient care come together. Position Highlights Monday–Friday | 8:00 AM–5:00 PM Shared call rotation In-person and virtual visits supported by Medical Assistant Manage complex patients with ongoing follow-up throughout stay Collaborate with SNF team to facilitate care coordination Implement discharge planning to improve patient outcomes Signing bonus, CME, and relocation available.
Requirements
- Physician Assistant or Nurse Practitioner
- Passion for working in a SNF/Post-Acute Care Environment
Nice To Haves
- Experience in a Skilled Nursing Facility is preferred but not required
