About The Position
Atrium Health is seeking Physician Assistants and Nurse Practitioners to join their Adult Medical Specialty team within the Post Acute Network — Skilled Nursing Facility (SNF) program in Charlotte, NC. This role is part of the SNF/Post-Acute Care Environment and involves managing complex patients with ongoing follow-up throughout their stay. The position offers a Monday-Friday schedule with shared call rotation, and in-person and virtual visits supported by a Medical Assistant. Clinicians will collaborate with the SNF team to facilitate care coordination and implement discharge planning to improve patient outcomes. The role emphasizes supportive leadership, strong work-life balance, and the opportunity to deliver exceptional patient care within a team-oriented group of compassionate clinicians.
Requirements
- Physician Assistant or Nurse Practitioner
- Passion for working in a SNF/Post-Acute Care Environment
Nice To Haves
- Experience in a Skilled Nursing Facility is preferred but not required
